Tarleton State University is now partnering with Med+Proctor for the submission of bacterial meningitis vaccination records. You are not required to pay a fee if you choose to “Submit your health forms for free without the upgrade“. You may also choose to pay $10 if you would like the extra benefits Med+Proctor provides.

Bacterial Meningitis

STUDENTS MAY BE UNABLE TO REGISTER FOR CLASSES UNTIL THIS REQUIREMENT HAS BEEN COMPLETED.

TEC 51.9192 requires all students, under the age of 22, entering an institution of higher education (public and private) to receive a bacterial meningitis vaccination or provide exemption/decline documentation before the first day of the semester. 

Once you have been admitted, register with Med+Proctor in order to complete and submit your bacterial meningitis vaccination record. You will use your Tarleton student email address to register. Admissions does not accept proof of vaccination. Students should follow the steps below, as the vaccination must be received at least 10 days before the first day of classes.

Evidence of vaccination must be submitted through Med+Proctor. There are no fees for submitting documentation unless you choose to do so for the services that Med+Proctor offers.

Students may be unable to register for classes until this requirement has been completed. Vaccination cannot be more than 5 years old on the first day of the semester. If the vaccination is expired, the vaccination must be 10 days prior to the first class day.

Exemptions

Students may be exempt if:

  • They are 22 or older on the first day of the semester.
  • Enrolled in an online degree program. In the MedProctor portal in myGateway under ‘Documents’ then ‘Download Documents’, the student will choose ‘Online Exemption Form’ and complete and upload the form in the MedProctor portal.
  • If a student cannot receive the vaccine because it would be injurious to your health and well-being, follow these steps:
    • Request a signed letter from your physician who is duly registered and licensed to practice medicine in the United States, in which it is stated that, in the physician’s opinion, the vaccination required would be injurious to the health and well-being of the student.
    • Submit this documentation to your Med+Proctor account.
  • If a student declines the vaccine due to reasons of conscience, including a religious belief, follow these steps:
    • Request the Texas Department of State Health Services (TDSHS) Vaccine Exemption Affidavit Form- Affidavit Request for Exemption from Immunizations for Reasons of Conscience.
    • This is the only acceptable documentation for this exemption, and this process takes a significant amount of time to receive from the state.
    • The community college form is not valid for universities.
    • Submit the original notarized affidavit with chosen vaccinations checked within 90 days of it being notarized to:
