Online registration is open for all Tarleton students and employees (ages 18 and up) who want to receive the Moderna COVID-19 vaccine.

To register, complete Tarleton's vaccination registration form. Completed forms will be reviewed and a university representative will contact you to confirm your appointment on the Stephenville campus. Appointments are required to protect participant privacy.
